# Bioavailability

If you’ve read up on CBD before - specifically different ways to take CBD - then you are probably familiar with the term ‘bioavailability’. Whether a newbie to CBD or an experienced user, we all want to know how to get the most out of the CBD products we are paying for. This is why bioavailability is an important thing to understand. 

Bioavailability heavily impacts the effectiveness of different drugs - CBD included - and how they work in the body. In this article, we will explain exactly what this means and how this applies to CBD products.

## What Does Bioavailability Mean?

In short, bioavailability refers to the amount of a substance that is absorbed into the bloodstream and the rate at which this occurs. When you take any drug, only a proportion can actually reach your bloodstream and have the desired effect. This is due to the processes that the substance must pass through in order to reach its final destination - i.e. the bloodstream. 

For example, if you take a pill orally, it must pass through the metabolic systems (the liver) and digestive systems which will filter out a fairly large proportion of that substance. So in order to maximise the bioavailability of a drug, you want a consumption method that will provide the most direct route to the bloodstream. 

## How Does This Relate to CBD Products?

When you take drugs from your pharmacist, doctor, or over-the-counter medications, the prescribed/recommended dosage already takes bioavailability into account. With CBD, there are so many different products on the market, all with varying methods of consumption. This causes the bioavailability to differ. 

This essentially means your optimum dosage will change depending on the type of product you are using. This is one of the reasons why it’s so difficult to recommend one ideal daily dosage for CBD users. If you want to get the most out of your CBD products, you should first consider bioavailability and which type of product might be the most effective. 

## So, What is the Best Way to Take CBD in Terms of Bioavailability?

### Sublingually (CBD Oils)

Many people would probably assume that CBD oils would have a similar bioavailability to CBD edibles and soft drinks. However, this is far from the truth! ‘Sublingually’ refers to the method of absorbing a product via the sublingual glands underneath the tongue. This allows the CBD to be absorbed across the thin membrane directly into the blood vessels. This is why it is so important to hold the CBD oil under your tongue for at least a minute before you swallow. 

CBD oils can therefore provide an excellent alternative to CBD edibles in terms of effectiveness. And if you’re disappointed that the tasty CBD sweets aren’t as effective as CBD oils - don’t worry! At Dr K, we have a delicious fruity lemon flavour and a fresh peppermint flavour for you to enjoy.

### Topically (CBD Skincare Products)

Using CBD topically is very different from using the other CBD products we have listed. When you apply the product to the skin, the CBD enters local tissue and small blood vessels only in that area - so it does not actually reach the full bloodstream. This means that CBD topicals are excellent for treating pain or discomfort in one specific area of the body without having an effect on the body as a whole. 

The bioavailability is fairly high since the product can be applied in a targetted area and absorbed into the skin quickly. At Dr K, we use nano-emulsified droplets in our CBD topicals. In short, this means that the CBD particles are tiny - much smaller than that of most topicals on the market. Therefore, the CBD has a much easier time absorbing deeply into the skin and providing quick and effective relief.

### Orally (CBD Edibles)

This is probably the least effective way to take CBD in terms of bioavailability. However, many CBD users absolutely love using edibles since they are so easy to take. Plus - CBD gummies, chocolates, and other tasty treats probably provide one of the most enjoyable consumption methods! 

As we briefly mentioned above, when we ingest CBD orally the CBD must pass through the metabolic and digestive systems before it can reach the bloodstream. This causes a large proportion of the CBD to be lost along the way.

### CBD Soft Drinks

In addition to CBD infused foods, you can also find CBD soft drinks on the market - one of the most common is CBD infused water. CBD soft drinks have similar effects to CBD edibles since they are consumed orally and must be digested and metabolised.

> Average time to feel effects: 30-120 minutes

### Vapourised (CBD E-Liquids)

CBD e-liquids provide one of the highest bioavailabilities of all CBD products. This is because the vapourised droplets are inhaled directly into the lungs. From here, they cross the thin mucus membrane directly into the bloodstream. Plus, the large surface area of the lungs means that this absorption happens very quickly and easily. 

However, we would not recommend vaping CBD (or anything for that matter) at Dr K. Unfortunately, the dangers of vaping seem far too real. Since vaping is a fairly new phenomenon, there is a lack of evidence and regulation to suggest that it is safe for us at all. And since smoking is so damaging to human health, it isn’t unwise to suggest that vaping could have similarly drastic implications.

### Intranasally (CBD Nasal Sprays)

Taking CBD intranasally can be just as effective as vaping it, but without the negative implications to our health. This involves spraying the product into the nostril and sniffing it into the lungs (you may be familiar with this method if you have used a cold & flu nasal spray before). 

Intranasal CBD products are far less common than the other products we have listed - possibly because it’s not a very enjoyable method of consumption!

# Extract vs Isolate vs Distillate

One thing that is important to understand when venturing into the world of CBD is the difference between these 3 things: CBD extract, CBD isolate, and CBD distillate. There are so many different products on the market that provide consumers with so many different options. This is why it’s good to know exactly what you’re looking at. 

In this article, we will explain the difference between these types of CBD so that you can understand exactly what you’re buying. This new understanding should help you decide which type of CBD will best suit your preferences. 

## CBD Extract

CBD extract - sometimes referred to as full-spectrum and broad spectrum CBD - contains everything all of the benefits of the whole hemp plant - cannabinoids, terpenes, and organic plant material. 

In the UK, the THC content of a CBD product must be below 0.3% or less than 1mg in a finished product. If it’s higher than this, the product will be illegal to sell and could potentially be dangerous. This is why it’s so important to buy from a reputable brand who can provide you with cannabinoid testing reports. Dr K CBD oils are THC free and non-psychoactive. 

Full-spectrum extract is great for those who want to experience the full entourage effect and aren’t concerned about the inclusion of THC.

## CBD Isolate

CBD isolate is the purest and most refined form of CBD. This type of extract contains no other cannabinoids, oils, terpenes, and any other organic hemp matter. It is simply the CBD compound which is 99.7% pure. 

During the extraction process, everything is filtered out apart from the Cannabidiol (CBD) compound. This creates a powder or crystalline form of CBD extract which can then be put into various CBD products, like oils, gummies, or creams. 

Due to the fact that all other organic matter is removed, CBD isolate is pretty much tasteless. This makes it a good option for those who want a product without any trace of the natural, earthy hemp flavour associated with other CBD extracts. However, this type of CBD does lack the advantages that a combination of cannabinoids and plant matter can provide - this is referred to as the entourage effect. We will discuss this in more detail below.

## CBD Distillate 
CBD distillate - sometimes referred to as broad-spectrum - is very different from CBD isolate. As well as the fundamental CBD, it can contain traces of other cannabinoids (e.g. CBG, CBC), terpenes, and other organic plant matter. 

Terpenes are a special type of oil that is found in most plants - they provide plants with their unique aromas and flavours. In addition to this, hemp terpenes are thought to provide a large and diverse array of benefits to human health. For example, the terpene Humulene is thought to be an effective anti-inflammatory, and Caryophyllene is thought to have antioxidant, antibacterial, and pain-relieving effects. 

This combination of powerful cannabinoids and terpenes is what creates the entourage effect, which we briefly mentioned above. The entourage effect occurs when lots of different compounds work together synergistically to boost the overall therapeutic effects of the extract. This can essentially mean that a product infused with CBD distillate is more powerful than a product infused with CBD isolate. 

The significant thing to note about CBD distillate is that it contains no THC. During the extraction process, the cannabinoid THC is filtered out completely. This is appealing to many people who want to avoid having THC in their system. THC is a psychoactive and controlled substance - although the amount consumed would be tiny, some people would prefer to avoid it altogether. 

So, CBD distillate is a great choice for those who want to utilise the power of the entourage effect without having to consume any THC. 

## Summary  
So, in summary:

<h1>FIBROMYALGIA: THE PERPETUALLY SOUNDING CAR ALARM</h1>

Managing fibromyalgia pain can be difficult; one of the main hurdles being that it is a condition that is poorly understood. What are the symptoms?

**Widespread generalised pain: ** This is ‘‘chronic ‘’,  meaning it has been going on for at least 3 months. The pain can be intermittent initially and later become more persistent. It need not always be in the same place. Indeed, people can have tender points in multiple locations throughout the body. The pain is often made worse by damper, colder weather conditions and stress. 

**Fatigue :** 90% of people report intractable tiredness.

Disturbed Sleep patterns: poor sleep quality leads to difficulty with daytime functioning, and has a knock on effect on pain and fatigue leading to a vicious cycle of waking unrefreshed. 

**Brain Fog : **Problems with  memory, recall, fluent speech and poor concentration are all reported. 

**Depression : **Depression and anxiety are seen in up to 75% of people.
Other symptoms include Irritable Bowel Syndrome, migraines and bladder disturbances. 

**The Car Alarm That Won’t Stop Going Off……**

No one really knows what causes Fibromyalgia and if they can use CBD oil for pain. One theory is that it due to ‘’altered central pain processing’’, causing a lower threshold of pain perception in the brain. Dr. K CBD oil for pain is able to alleviate this pain slowly and gradually.

CBD Oil for pain: How do we System Reset?

The treatment of Fibromaylgia is very specific to the individual, however a few approaches have been shown to work. 

Physical Therapy : Studies have shown that regular aerobic exercise ( at least 20 min /day, 2-3 times per week  for at least 3 weeks ) improves pain and general wellbeing compared with no exercise.  This could be anything that gets your heart rate up such as walking, jogging or swimming.  Heated pool or spa treatments can also be effective. 

Acupuncture: this is not really proven but could work for some 

Psychological Therapy : Helping people to fully understand the condition and also psychological intervention such as Cognitive Behavioural Therapy  has also been shown to help.

Could CBD Oil for pain help with Fibromyalgia?

Emerging data from trials does suggest that CBD oil for pain i.e. cannabis has a beneficial effect in people with Fibromyalgia. A publication in Best Practice Research in Clinical Anaesthesiology states that retrospective trials and patient surveys suggest significant reduction in pain, improvement of sleep and other associated symptoms. (1)

A 2011 study looked at 56 people with Fibromyalgia, half of which used cannabis and half of which did not. The cannabis users experienced benefits such as reduced pain and stiffness and an increase in sleepiness 2 hours after use (2). 

A 2009 review concluded that CBD provides effective relief of ‘neuropathic’ pain (3) , pain affecting the nerve cells. People with Fibromyalgia have nerve endings that are thought to be ‘oversensitive’, so it’s entirely feasible that CBD oil for pain could dampen down the response.

Some of these studies looked at cannabis, others at CBD but what’s the difference?

Your GP Can Also Help……

CBD Oil for pain: There are a number of prescription and over the counter medications that can improve symptoms of Fibromyalgia. 

Paracetamol and Non Steroidal Anti inflammatory Drugs ( such as Ibuprofen) can reduce pain. These can be purchased over the counter. 

Opiods : There is evidence that Tramadol can reduce pain in those with moderate to severe symptoms that haven’t responded to other treatments. 

Antidepressants : Studies have shown that antidepressant medications can improve pain, fatigue , depression and disturbed sleep. 

Other pain medications such as Gabapentin and Pregabalin could also improve symptoms ,speak to your GP for further guidance.

**Travelling with CBD in America. **

You may have already heard horror stories surrounding people getting arrested when carrying CBD products into the state - though many are fabricated or exaggerated. Similar to European Law, you can carry CBD products with you, so long as their THC content is relatively low. In many areas of the US, they are slightly more lenient than in Europe, allowing for a THC content of 0.3%. If you use a CBD product for medicinal purposes, it must be FDA approved.

**Travelling with CBD in Australia.** 

CBD products are entirely legalised in Australia, meaning that you should have no qualms bringing them along with you when travelling. However, you should only carry small amounts for personal use. 

In short, if you are unsure if you can travel with CBD products, ensure that you do your research beforehand. Each country has different laws surrounding the use of CBD products, and you must understand them before heading to the airport as you don’t want to land yourself in a tricky situation. 

Regardless as to where you may be travelling,  you need to ensure that you have proof that your product is derived from hemp. This should again be detailed on the labelling, or online product description. If you have any further worries or qualms, you should contact your airline before traveling, who will be able to provide you with more specific advice. For example, they can let you know whether you should store your CBD products in your suitcase, or whether they are allowed in your carry on.

The Red Haze….
PMS or Premenstrual Syndrome can give rise to distressing psychological and physical symptoms, which usually occur a few to several days before the period.  Physical symptoms include headaches, breast tenderness, bloating and weight gain, joint pains, back pain, skin problems and swelling of the hands and feet. 
Many women report feeling dejected and down, also displaying other psychological symptoms such as feeling irritable, angry, hopeless, anxious, and having poor concentration and disturbed sleep. Symptoms can be debilitating, with up to 10% of women reporting them as severe, and 13% of women having to take time off work.
And let’s not forget period pains. No one knows exactly why periods are painful, but it’s thought to be linked to a group of fatty  molecules known as prostaglandins, which act on the muscle in the wall of the uterus causing it to contract more. Typically the pain starts 24 hrs before the period, lasts 2-3 days and is felt in the lower abdomen, back and thighs.

The Storm before the Calm….
The cause of PMS is thought to be due to a combination of certain women being more sensitive to the female hormone progesterone and also due to the effects of this hormone and also oestrogen on the serotonin and GABAA receptors in the brain. These receptors are thought to regulate mood and cognition. 
Could CBD help with PMS?
Evidence suggests that CBD can alleviate pain and modulate pain pathways (1). This leads us to believe that it could improve symptoms in PMS, however further research and clarification is needed. 
A review from Frontiers of Immunology suggests that cannabidiol (CBD) could help protect nerve cells which could stabilise mood (2) As we know, irritability, anxiety and depression can all be symptoms of PMS. 
An article published in the Neurotherapeutics journal noted that CBD might have anti-inflammatory, anti-anxiety, and sleep-promoting effects (3). Joint pains and insomnia are common symptoms of PMS.


Lifting the Red Haze….
There are other ways to improve the situation…
Lifestyle measures such as exercise and taking certain supplements such as Vitamin B6 may be enough to improve symptoms for some. 
The Combined Oral Contraceptive Pill (COCP) can be very effective at reducing both the physical and psychological symptoms and usually have the added benefit of making periods lighter and less painful. These can be taken with or without a break. 
SSRIs or Selective Serotonin Receptor Inhibitor are a type of antidepressant that work by increasing levels of serotonin, the ‘feel good hormone’. This can be taken on a continuous basis or on just during the second half of your cycle. 
These medications need to be prescribed your GP who will need to discuss their risks and possible side effects. In addition, if these treatments do not work they may be able to discuss other options or refer you to a Gynaecologist.

The variation of these patterns is what causes women to release an egg every month and enables them to ultimately bear children. On the flip side of the coin are period pains, breast tenderness, bloating and mood changes, which rear their ugly heads due to subtle shifts in the hormonal cycle. 

PMS, MENTRUAL CRAMPS AND ENDOMETRIOSIS :  HOW CAN WE LIFT THE RED HAZE?

This is a painful bladder condition that results in a frequent, urgent need to pass urine. There is a constant need to pass urine throughout the night and day. This may be accompanied by chronic pelvic pain, pain in the region of the bladder before and after passing urine, and pain during sexual intercourse. Its much more common in women than in men. 

WHAT CAUSES IT? 
Inflammation may play a role. 5-20 % of patients with this condition will have prominent blood vessels on the bladder wall together with ulcers, scars and a reduced bladder capacity. 
The inflammation is thought to be caused by an autoimmune process, when for unknown reasons, the human immune system attacks its own organs. This may damage the bladder lining, allowing toxins from the urine to seep into the bladder wall, triggering the symptoms. 
In people without these changes, other factors may come into play. Your bladder has receptors in its walls, when they become stretched, this signals to your brain that you need to pass urine.  In some people these receptors may be ‘oversensitive’ and send out constant signals to your brain that you need to pass urine, when, actually your bladder is nowhere near full. There are similar processes going on in conditions such as Fibromyalgia and Irritable Bowel Syndrome and these conditions may overlap. 

CAN CBD HELP ME? 
Interstitial Cystitis is a condition of exclusion, this means a specialist needs to perform a cystoscopy to examine the inside of your bladder to make sure there is nothing more serious causing your symptoms.

ARE THERE OTHER TREATMENTS?
Oral treatments include simple painkillers such as paracetamol, Non Steroidal Anti-Inflammatories such as ibuprofen and some types of antihistamine. Your GP could also prescribe medication to treat neuropathic pain (affecting the nerves) such as amitriptyline, gabapentin and pregabalin. 
Another treatment involves directly introducing liquid medication into the bladder via a plastic tube. The use of the bacteria BCG in this way has been shown to improve bladder symptoms by up to 60 %. If other treatments have not worked for you, it may be worth approaching your GP to discuss this option.

WHAT IS INTERSTITIAL CYSTITIS ?

What is the menopause? 
Menopause, or ‘the climacteric’ is a transition that all women will go through at some point in their lives. The time leading up to the point when your periods completely stop is known as the perimenopause and during this time women may experience a multitude of symptoms both physical and psychological.
 In this blog we will discuss the main symptoms, why you feel the way you do, and hopefully provide some strategies to help you adjust to the new you. 
Why does it happen?
Every month your ovaries release an egg, which produces the reproductive hormones oestrogen and progesterone. In the perimenopause the number of eggs released becomes fewer, the falling hormone levels themselves have dramatic effects on many systems in your body and they also contribute to a cascade reaction of a hormone from higher centres in the brain that is thought to cause one of the most troubling symptom, hot flushes. 
 
What are the main symptoms?

Hot flushes affect 85 % of women, can be as often as every half an hour and can be accompanied by chills, fluttering of the heartbeat and profuse sweating, particularly at night. For the majority of women, the flushes gradually get better and are gone within 4-5 years.  A quarter of women may have symptoms for more than 5 years. 
Can CBD Relieve Hot Flushes ?
A study published at the 2020 Meeting of the North American Menopause Society showed that 1 in 4 women were using CBD to ease symptoms of Menopause.
In the laboratory, it has been shown that Anandamide, an endogenous Cannabinoid that occurs naturally in your body, is produced by nerve cells in the brain’s higher regulatory centres.(1) These have been shown to affect the pulsatile release of hormones that are crucial in the reproductive cycle. However, more studies are needed to fully understand the relationship between CBD and menopausal flushes. 
Vaginal Dryness
Lack of oestrogen causes vaginal dryness that can make intercourse painful. It also causes muscle in the wall of your bladder to become less stretchy, which can cause a constant urge to pass urine that can also be uncomfortable. 
Oestrogen therapy in the form of a tablet, ring or pessary inserted into the vagina can considerably improve symptoms of vaginal dryness, itching and problems with passing urine, and because the hormone is not widely absorbed into the bloodstream, is associated with significantly lower risks than conventional HRT.

Getting Back Your Mojo….
Many women experience reduced libido during the perimenopause. Studies have shown that higher concentrations of naturally occurring cannabinoids are associated with increased female sexual arousal, hence CBD could improve libido in menopausal women.

Insomnia is common during this time, partly due to the physical symptoms but also due to the psychological effect that hormonal fluctuations can have.

Your GP Can Help……
Communicating with your GP is an important part of managing menopausal symptoms. They can advise you about a range of different prescription medications which may improve the situation, as well as screen for other medical conditions such as osteoporosis and heart disease which become a greater concern after the menopause. 
The average age of a woman going through menopause in the UK is 51 years, although it can occur from 40 -58 years of age.   In most cases blood tests are not needed to confirm the menopause, but your GP can arrange one if you are in the younger age bracket and the diagnosis is not clear cut. 
HRT
HRT is hormone replacement therapy, it aims to replace oestrogen that is lacking in postmenopausal women and hopefully reverse some of the effects that lack of oestrogen causes. It can be taken in the form of tablets, patches, or gels. 
It must always be prescribed by a doctor who will need to discuss the risks associated with taking it. The most important ones are, an increased risk of breast cancer, increased risk of blood clots or DVT and in some women, an increased risk of stroke. 
I don’t want to take hormones…….
In women who want to avoid hormonal treatment, there are other options available. Medications such as Clonidine, Gabapentin and Pregabalin have been shown to reduce hot flushes and in the case of the latter two may also help with joint pains.  
Antidepressants such as Paroxetine and Venlafaxine can also improve hot flushes and improve quality of life in about 20-50% of cases 

Are there other options other than prescribed medication? 

There are a range of complementary therapies on the market that are aimed at treating menopausal symptoms in women. 
Oil of evening primrose is used to treat breast tenderness. There is some evidence that isoflavones or black cohosh may relieve hot flushes. However, these preparations are not tested and regulated in the same way prescription medications are, which means the ingredients will vary from brand to brand as will their effectiveness and safety profile. They can also interact with other medications. 
Psychological support in the form of Cognitive Behavioural Therapy can improve anxiety and low mood associated with the menopause, consult your GP for further information about this.

The time leading up to the point when your periods completely stop is known as the perimenopause and during this time women may experience a multitude of symptoms both physical and psychological.

Vulvodynia is the presence of chronic pain in the vulva, characterised as a chronic rawness, irritation, burning and soreness with no identifiable physical cause. 
Ok, so what’s the next step? 
Vulvodynia can be primary, without a physical cause, or secondary. It is important to visit your family doctor and have an internal examination, to exclude secondary causes such as skin conditions, an infection, or a structural problem.

There are many factors that play a part in Vulvodynia.
Anxiety and Tight muscles
Psychological factors certainly contribute; sometimes just the anticipation of discomfort during sex can cause a tightening of the vaginal walls that is painful, a condition known as Vaginismus.  This makes intercourse almost unachievable, further compounding anxiety and intimacy issues.  
Inflammation and Pain 
In some women with Vulvodynia, a certain part of the vulva known as the vestibule (the part just below the clitoris), can become red, swollen, and irritated. In this case the pain is not usually constant, it is mainly generated by touch and pressure to this area.
Constant, searing, burning pain is usually neuropathic pain. This occurs when nerve fibres which are meant to trigger when they receive a painful stimulus such as a pin prick, signal back to your brain constantly, and also misinterpret non painful stimuli such as touch and pressure as painful ones.

Are there other treatments? 
Other treatments include….
Creams: these can be anti-inflammatory, anaesthetic / numbing creams or localised oestrogen therapy. 

Tablets: these can be painkillers, either simple or anti-inflammatory or oral medication prescribed to treat neuropathic pain. 

Biofeedback: This assumes that pain is caused by unstable muscles in the pelvic wall which contribute to the shooting and stabbing pains. This approach uses electrical stimulation to retrain the muscles. Pelvic floor exercises also work in this way. 

Psychological Support: Anxiety and stress management, as well as challenging negative beliefs through Cognitive Behavioural Therapy could also produce results. 

Acupuncture: some women find relief of symptoms with this, although this is not usually a long- term solution.

So, how does it work?
